A different approach...
Spectrum Healthcare Solutions is a network of providers who are committed to improving transitional care and shrinking gaps in care. Although Oklahoma is currently far behind national trends for 30-day readmission rates, we at Spectrum are dedicated to changing this trend.
Because our providers work daily in acute-care hospital settings, they are uniquely qualified to bridge the gap between acute and post-acute care settings, which enables a smooth transition of care and limits negative outcomes during these vulnerable transitions.
While patients are being cared for in the hospital, they are assessed daily by a variety of healthcare providers, but upon returning to their communities from in-patient facilities, their care often suffers. The typical "skilled nursing model" in Oklahoma generally relies on a single provider who is only on-site once a week. Unfortunately, this approach often results in service that is slow to address emerging concerns and perpetuates care gaps, which are stressful to both patients and the staff who care for them.
Our innovative model ensures patients are seen in a timely manner and any transition of care concerns are addressed promptly. To accomplish this goal, Spectrum has pioneered innovative ways to improve patient outcomes and reduce the number of patients who return to acute care settings by implementing key, data-driven interventions.

Nurse Liaisons facilitate communication between facilities and providers, and ensure a smooth transition of care and prompt interchange between care providers.

Nurse Practitioners and Physician Associates round daily in facilities to address both emergent and non- emergent needs in a timely manner.

Physicians with in-patient, acute care experience are on-site weekly to provide care oversight by addressing emergent concerns and ensuring optimal service delivery. Because our doctors work in hospitals on a daily basis, they bring specialized acute-care insights and are uniquely equipped to aid patients in returning home after stays in post-acute care settings.

Spectrum's network of specialty consults continues to grow and allows for prompt intervention by experts in the fields of cardiology, infectious disease, nephrology, endocrinology and mental health services.

Spectrum provides monthly in-services to nursing and/or support staff, to increase disease process knowledge and promote the highest quality of care. These trainings seek to address site-specific concerns, which are identified by analyzing tends found within the prior month's aggregate data. In this way, Spectrum seeks to deliver data-driven care that meets each location's specific concerns with an individualized, timely approach.

Our goal is to ensure the rate of return to acute care (RTA) is less than 15%, which is well below the national average of 22.9%, as well as the Oklahoma's average of 25.2%. By implementing our data-driven approach, the facilities we serve boast an average RTA rate of 11%. Although we are very proud of our facilities' RTA rates, our true passion is providing high-quality and effective medical care by combining compassion, specialized acute-care expertise, and data-driven interventions to ensure the rapid achievement of positive outcomes within the post-acute care setting. This approach empowers patients to return home in both a safe and timely manner.
